Your Pet’s Health Log helps organize care. It does not replace veterinary care.
The public trackers, worksheets, and printable PDFs are free tools for recording observations and preparing clearer notes for veterinary conversations.
Organization, not diagnosis or treatment
Your Pet’s Health Log does not diagnose disease, prescribe medication, recommend doses, establish treatment targets, or provide veterinary medical advice. Use the care plan provided by a veterinarian who knows the animal and their circumstances.
Not an emergency service
Do not use a tracker to delay care. For urgent, severe, rapidly changing, or concerning symptoms, contact a veterinarian or veterinary emergency service promptly.
Use the paperwork as a record-keeping aid
The fields and prompts are designed to help organize observations. They may not cover every condition, species, symptom, measurement, or question that matters for a particular animal. Follow instructions from the veterinary team when they differ from a Your Pet’s Health Log prompt.
Availability and accuracy
Your Pet’s Health Log aims to keep the site useful, accessible, and accurate, but the site and downloads are provided as available and may be changed, corrected, expanded, or removed as the project improves.
Support is optional
Every public Your Pet’s Health Log tracker is usable without payment, an account, or an email address. Buy Me a Coffee is an optional third-party support link; using it does not change access to the trackers or the information they provide.
